To book an appointment, a valid credit card is required for fees associated with the 48-hour Cancellation Policy (see below). In addition, if your insurance denies a claim for any reason or you have a cost share (such as a copay, coinsurance, or deductible) you would be responsible for paying any fees not covered by insurance.
I will never charge your card without letting you know first. But I do keep a card on file to protect myself against scenarios in which individuals don’t show up for an appointment or have a claim denied, and subsequently don’t pay money owed for services I provided.
Please remember to cancel or reschedule at least 48 hours in advance. Cancelling or rescheduling with less than 48 hours notice will be considered a “late cancellation”. An appointment is considered a “no-show” if you are 15 or more minutes late. You will be responsible for 100% of the session fee (based on self-pay rates) for both late cancellations and no-shows. This fee is not covered by your insurance and your insurance copay does not apply. By booking an appointment, you agree to be responsible for paying these fees via your credit card on file.

Before our first session, you will need to complete required intake form(s). Upon booking an appointment, you will receive an email with a link to complete the form(s). The email will be sent from Carepatron. I typically request these forms be completed at least 24 hours in advance of your appointment. The forms include a short questionnaire, practice policies, consents for receiving services, credit card authorization, financial agreement, and HIPAA notice of privacy practices. If you are planning to use health insurance, you will complete an insurance intake form as well as upload front and back images/copies of your government-issued photo ID (typically a drivers license or state ID card) for identity verification purposes.
Our first session will last between 55-85 minutes. It is all about getting to know one another. We will talk about you: your health and nutrition history, your lifestyle, what has worked (or hasn't), the reason for scheduling your appointment and what is most important to you. I want to hear your story. After learning more about your background and after some initial discussion, I will provide some nutrition education (if needed), and we'll focus on small, sustainable steps and strategies to achieve your goals.
The first nutrition session may look a little bit different for everyone. Everything I do is personalized for the individual I'm working with. I will use my knowledge and experience to help you set short-term, achievable goals you feel good about. You don't need to have everything figured out. I'm here to support you.
If you're using in-network insurance, I will submit a claim for our session on your behalf. If your health insurance covers my services, you won’t need to do anything. If your health insurance company denies the claim, or says that you owe money, I will provide you with an invoice. I will never bill your credit card without letting you know beforehand, along with the reason for the charge.
The goal is for services to be covered in full by your insurance, and for my clients to have to pay as little as possible out-of-pocket. This doesn’t always happen, as coverage varies between plans and insurance companies, but I do my best to get services covered and to minimize out-of-pocket costs as much as possible.
You are responsible for verifying your benefits and understanding the details of your insurance coverage. I've created a step-by-step guide to help you verify your insurance coverage. In order to assist you I will need your insurance information, including clear font and back photos of your insurance card and your date of birth.
After our session, I will send you an invoice for the full session amount based on self-pay rates. Once the invoice is paid in full, I will provide you with a superbill (a detailed, itemized receipt) that you may then submit to your insurance company for potential reimbursement. However, I cannot guarantee that your insurance plan will reimburse you, even with a superbill. Please call your insurance provider to verify out-of-network benefits prior to our appointment.
No. You can pay my self-pay rates if you would prefer not to use your health insurance, if your health insurance doesn’t cover my services, or if you’re outside of the U.S. Ask me about my 20% off prompt-pay discount for clients using self-pay rates at the time of service.
It's up to you! The majority of clients opt to schedule a session with me every other week to keep themselves accountable. I have a few clients who meet with me weekly for added accountability and a few clients who meet with me every 3-4 weeks. Some clients just prefer to have one session.
