Instructions for Checking Insurance Benefits
In an effort to streamline the process of billing insurance for services, we are asking you to call your own insurance carrier to inquire about the details of your benefits.
Your insurance may cover some or all of your nutrition visits. Be sure to ask your insurer about Preventative Benefits when you call. Preventative services are typically covered in full with no deductible or copay. It is your responsibility to find out what they will cover, and it is your responsibility to pay if your insurance does not.

You will want to have the following info ready when you make the call:
Client Name
Client Date of Birth
Member ID (from your insurance card)

What to Ask Your Insurer
Do I have nutritional counseling coverage with the following CPT codes: 97802 & 97803
If they say you do not have coverage using those codes, ask them about 99401, 99402, 99403, & 99404. We can also bill for S9470 if it is covered by your policy.
Do I have any nutrition counseling coverage under the preventative care portion of my plan?
Will my diagnosis be covered?
Use the diagnosis code Z71.3. If they don’t accept Z71.3, then provide them with Z72.4
If you are overweight, obese, have pre-diabetes, diabetes, hypertension, high cholesterol, or a family history of cardiovascular disease (CVD) you may want to see what your coverage is for these diagnoses as well.
Confirm whether these are covered under preventative care or regular medical benefits.
Do I have a cost-share for my nutrition visit? (e.g. deductible, copay, coinsurance)
Do I need a referral to see a Registered Dietitian?
Is pre-authorization required? If so, what is the process?
Is there a visit limit or a unit per visit limit?
Is telehealth covered? Is the cost-share the same as an in-person visit or will my responsibility be different?
Can I have your name and a reference number?
Document the date & time of the call for future reference.
** Unfortunately, insurance never guarantees benefits over the phone. If a claim is denied, we will attempt to get the claim reprocessed. However, if the information that was provided to you was incorrect or incomplete, then you will be responsible for the balance for the service.
